⚙️ Developer Setup Note — Remove Before Launch

This form is designed to connect to Google Forms + Zapier for automation. To go live:

Create a Google Form with all fields below (First Name, Last Name, Email, Phone, Business Name, Business Address, Birthday, How Did You Hear, File Upload, T&C checkbox).
Publish the Google Form and copy its action URL from the form's source code.
Replace YOUR_GOOGLE_FORM_ACTION_URL in this page's script with that URL.
In Zapier, create a Zap: Trigger = New Google Form Response → Action = Send Email (notify trade@kathyadams.com) + optionally add to a Google Sheet or CRM.
For file uploads, use JotForm or Tally instead of Google Forms (Google Forms does not support file uploads via embed). Zapier connects to both.
Once connected, hide or delete this notice by removing the #setup-notice div.
